We are GROWING and have new positions available to expand our Primary Care Action Teams. Thames Valley Family Health Team is looking for many Patient Care Assistants for Oxford County and Middlesex London. The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. Currently, with more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care. The Thames Valley Family Health Team is one of the largest family health teams in Ontario. Currently, we are seeking skilled Patient Care Assistants to join our Primary Care Action Teams. This position will be a vital member of our team and will play a crucial role in providing comprehensive healthcare services alongside Nurse Practitioners (NPs), who serve as the most responsible provider(s), and the interdisciplinary healthcare team. The Medical Clinic Administrator responsibilities will include assisting with patient care, managing administrative tasks, and facilitating seamless communication within the team. The role will require some evening hours, with vacation aligned with the NP team schedule, and follow TVFHT Health and Safety policies and procedures. This Patient Care Assistant is a new role that will be working on-site with the multi-disciplinary healthcare team and TVFHT’s Operations Department to offer administrative and clinical patient support in a primary care setting. Requirements, Skills, and Abilities:
