Registered Pharmacist (Fulltime, ongoing contract)

We are GROWING and have new positions available to expand our Primary Care Action Teams. Thames Valley Family Health Team is looking for a Registered Pharmacist for Oxford County and Middlesex London. The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario, with more than 125 physicians across 20 plus sites within London/Middlesex, Oxford and Elgin counties. Some FHT sites are academic teaching sites affiliated with the Department of Family Medicine at the Schulich School of Medicine & Dentistry, Western University. The successful candidate will be expected to contribute to the teaching and mentorship of learners, supporting interprofessional education within a team-based primary care setting. . The FHT is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities.

Family Health Team staff includes nurse practitioners, nurses, social workers, pharmacists, dietitians and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centered care. Role Summary:

Pharmacists are integral members of the health care team and experts in medication management. Within a collaborative, patient-centered model, the pharmacist provides comprehensive pharmaceutical care, with a primary focus on direct patient care, identifying, assessing, and resolving medication related concerns. The pharmacist also serves as a trusted source of drug information for the team, community partners, and the broader community, supporting clinical education and knowledge translation. In addition, the pharmacist may contribute to system level work, including academic detailing, to support evidence informed prescribing and practice improvement across primary care.

REQUIREMENTS:

  • Bachelor of Science in Pharmacy / Entry-to-practice Doctor of Pharmacy (PharmD) degree from a from an accredited university program (Accredited program)
  • Two years’ experience preferred, or community/hospital residency and/or Pharm D an asset
  • Post BSc(Pharm), Doctor of Pharmacy degree asset
  • Registration with the Ontario College of Pharmacists (Part A)
  • Previous Primary Care experience in FHT setting
  • Previous academic Detailer experience preferred, or willingness to train and maintain a conflict of interest free approach
  • Certifications and training considered assets: Certified Diabetes Educator (CDE), Certified Respiratory Educator (CRE), Mint Memory Clinic training

SKILLS/ABILITIES:

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Large focus on medication reconciliation and assessment for high-risk patients (e.g. those with multiple chronic diseases, on > 5 chronic prescription medications, post-hospital discharge) with the goal of optimizing the health outcomes of the communities we serve
  • Improve accuracy of the medication profile in the EMR as appropriate
  • Use medical directives to facilitate patient care (e.g. smoking, diabetes)
  • Facilitate education groups and encourage the use of patient groups in a stepped-care approach to service delivery to optimize the care of the FHT patients and beyond
  • To provide un-biased evidence-informed education to FHT colleagues, Staff Faculty, and learners
  • To participate in quality improvement strategies
  • Demonstrated leadership, organization, research, evaluation, time management and interpersonal skills
  • Experience and commitment to working collaboratively within a interdisciplinary team an asset
  • Demonstrated computer literacy is required to facilitate the use of electronic medical records
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
  • Strong communication skills
  • Exhibit an ability to be open and non-judgmental
  • Knowledge of the goals and structure of the organization
  • Demonstrated ability in problem solving using sound judgment and critical thinking
  • Experience in providing patient-centered care and evidence-based unbiased drug information
  • Experience in program development, implementation, monitoring and evaluation
  • Proof of professional liability insurance

Hourly Rate: $45.5689 - $55.5718 (based on years of experience)
